The LP version includes the CD in a simple plastic sleeve. The vinyl itself is in a simple black sleeve:

The CD comes in a digipak with no booklet, just the credits on the inside:

CD Japan (!K7CDJ328):
The
CD comes in a jewel case inside a plastic "bag" with a sticker, it also
has the usual OBI strip (which has a download code for the bonus
track inside). The CD, front and back cover is the same as the European
version (it even says "made in the EU" on the back). It has a small
booklet, but it's similar to the European issue, so I didn't include it
here:
